This is a reflective hear in relation to an incident which occurred during my scratch line ward placement. Reflection is a key skill for maintains and can be an central learning tool (Wilding, 2008). For this reflection I will wont Gibbs (1988) model as it is fairly straightforward and can be adapted to numerous situations. The cycle has 6 stages firstly to carve up the story, then to con berthr how you felt, then what were the thoughts and feelings of others involved and how you knew these, followed by what were your options and what would you do if the situation arose again. The following account will go formulate each one of these stages.

I encountered a tolerant who was admitted with confusion. For the first 24 hours he would just wander around the ward and sometimes sit at the entrance to the bay without make any problems to either the staff or the other patients. Up to this point he didnt show any signs of assault or frustration and posed no threat. The next mean solar day there were marked differences in the patient, he had trouble with his equilibrize and was extremely unsteady on his feet, so much so that he fell over next to the nurses station and free burning a bump to his head.



Due to this change it was decided that he was a danger to both himself and others due to the high endangerment of falls.
The ward manager arranged for the patient to be travel onto a different ward where there was an empty side room, the furniture would be removed and mattresses placed on the base of operations so that he could lay on them without any essay of falling. He required a nurse to special him plainly this person was not available for a couple of hours so the ward manager requested that myself and another student nurse (also on her first placement) accompany him and ensure he came to no harm until the consultant could assess him.
